Release & Waiver of Liability

 

I understand that participating in sports and fitness activities involves inherent risks, including the potential for serious injury. I acknowledge that in any sport or fitness activity, including those offered by SAHR Sports LLC, participants may be seriously injured.

 

For Youth Participants:
I am aware that the dangers and risks associated with my child’s/ward’s participation in the workshop or camp include, but are not limited to, falls, contact or collisions with other participants, equipment, and facilities, as well as environmental factors such as high heat and humidity (facilities are not air-conditioned). I acknowledge that my child/ward may sustain serious injuries, including paralysis or death, as a result of these risks.

 

By selecting the "I Agree" checkbox above, I certify to SAHR Sports LLC that my child/ward is in good health, in sufficient physical condition, and able to participate in the sports workshop/camp.

 

I understand that SAHR Sports LLC recommends that my child/ward obtain a physical examination to identify any medical conditions or limitations that could affect their participation. I have informed the SAHR Sports team of any medical restrictions affecting my child/ward’s ability to participate.

 

Having been informed of the potential risks associated with participation, and in consideration of my child/ward being allowed to take part in the workshop/camp, I, on behalf of myself, my child/ward, my family, and our heirs, executors, administrators, and assigns, hereby assume all risks and agree to waive, release, discharge, and hold harmless SAHR Sports LLC, its employees, agents, representatives, physicians, athletic trainers, and volunteers from any and all liability, claims, causes of action, or demands for personal injury, death, or property damage arising out of or in any way connected to my child/ward’s participation in the workshop/camp.

 

For Adult Participants:
I understand that my participation in any SAHR Sports LLC adult fitness class, training, recreational league, or sports program involves inherent risks, including but not limited to physical exertion, falls, collisions, use of equipment, and environmental conditions such as heat or cold (facilities may not be air-conditioned). These risks could result in serious injury, illness, or even death.

 

I certify that I am voluntarily participating in these activities and that I am in good health and physical condition to engage in fitness and sports-related activities. I understand that SAHR Sports LLC recommends obtaining a physical examination prior to participation, and I accept full responsibility for any health risks associated with my participation.

 

In consideration of being allowed to participate in any adult program offered by SAHR Sports LLC, I, on behalf of myself, my family, and my heirs, executors, administrators, and assigns, assume all risks and agree to waive, release, discharge, and hold harmless SAHR Sports LLC, its employees, agents, representatives, physicians, athletic trainers, and volunteers from any and all liability, claims, causes of action, or demands for personal injury, death, or property damage arising out of or in any way connected to my participation in these adult programs.

 

This Release & Waiver extends to all claims of any nature, whether foreseen or unforeseen, known or unknown. The terms herein serve as an assumption of risk, release, and waiver for myself, my child/ward, and anyone else who may assert a claim on our behalf.

Informed Consent, Assumption of Risk & Release

 

For Youth Participants:
I acknowledge that my child(ren) is/are participating in the SAHR Sports Workshop/Camp. I understand that participation in sports and recreational activities involves inherent risks, including the risk of injury.

 

I also understand that SAHR Sports staff reserve the right to remove my child(ren) from activities in the building or on the field due to behavioral issues that may arise.

 

With full knowledge of these risks and in consideration of my child(ren) being permitted to participate, I agree to waive, release, and discharge SAHR Sports LLC, its employees, agents, trustees, and officers from any and all claims, demands, or actions of any kind or nature that may be brought by me or anyone acting on my behalf. This includes claims arising from my child(ren)’s participation in SAHR Sports activities, use of services, equipment, or facilities, and applies notwithstanding any negligence on the part of SAHR Sports LLC, its trustees, officers, employees, or agents.

 

I understand that it is my responsibility to become familiar with all SAHR Sports policies. Failure to comply may result in the suspension of privileges at SAHR Sports Workshops/Camps.

 

Additionally, I authorize SAHR Sports LLC and its agents to seek emergency medical treatment for my child(ren) if necessary to ensure their well-being. I certify that my child(ren) is/are in good health and physically able to participate in all SAHR Sports Workshop/Camp activities.

 

For Adult Participants:
I acknowledge that I am voluntarily participating in SAHR Sports LLC adult fitness classes, training sessions, or sports programs. I understand that participation in these activities involves inherent risks, including, but not limited to, muscle strain, falls, collisions, use of equipment, and environmental conditions. I recognize that injuries or medical complications, including serious injury or death, can occur as a result of physical activity.

 

I certify that I am in good health and physically capable of safely participating in these activities. I understand that it is my responsibility to consult with a physician regarding any health conditions or limitations that may affect my participation in physical activity.

 

With full knowledge of these risks and in consideration of being permitted to participate, I agree to waive, release, and discharge SAHR Sports LLC, its employees, agents, trustees, and officers from any and all claims, demands, or actions of any kind or nature that may be brought by me or anyone acting on my behalf. This includes claims arising from my participation in SAHR Sports activities, use of services, equipment, or facilities, and applies notwithstanding any negligence on the part of SAHR Sports LLC, its trustees, officers, employees, or agents.

 

I understand that it is my responsibility to become familiar with all SAHR Sports policies. Failure to comply may result in the suspension of my privileges at SAHR Sports classes or programs.

 

In the event of a medical emergency, I authorize SAHR Sports LLC and its agents to take reasonable steps to seek emergency medical treatment on my behalf.
